Community Integrated Health Services

Overview      

The Community Integrated Health Services (CIHS) mission is to assist individuals with mental health and/or SUD in obtaining the support, skills, and services necessary to improve their level of functioning within the Great Rivers five counties: Cowlitz, Lewis, Grays Harbor, Pacific, and Wahkiakum County. The services are delivered in a variety of settings including and not limited to in the home, community based settings, or office depending on the needs of the individual. CIHS Team Members must demonstrate a commitment to the provision of compassionate, recovery and patient-centered care supportive of the patient’s own goals and motivation toward positive health outcomes.

Services We Provide:

  • Flexible Assertive Community Treatment (FACT)- The goal of the flexible Assertive Community Treatment (ACT) Program is to assist individuals in obtaining the support, skills, and services necessary to improve their level of functioning within Cowlitz, Lewis and Grays Harbor County, and to decrease the need for hospitalization. The FACT Program is designed to serve high utilizers of multiple systems (HUMS) with LOCUS levels of 3 or 4. The services FACT provide are evidence-based, person-centered, recovery-oriented and delivered via a team approach.
  • WISe- Wraparound with Intensive Services (WISe) is designed to provide comprehensive behavioral health services and supports to Medicaid eligible youths up to 21 years of age (and their families) with complex behavioral health needs.
  • Regional Stabilization Team (RST)- The Regional Stabilization Team (RST) is a specialized form of crisis assessment and intervention designed to provide community-based alternatives to psychiatric hospitalization. Our wrap-around framework combines extended family, community and natural resources and more traditional mental health services to bring about safety, stabilization and community integration. The program is available 24/7 in CIHS’s full service area serving participants with LOCUS level 5.
  • Trueblood/Diversion- The goal of the Diversion Program is to assist individuals in obtaining the support, skills and services necessary to improve their level of functioning within their communities and to decrease the need for hospitalization and correctional settings within the Great Rivers BHO Provider Network. The services are delivered in a variety of settings, predominately in the community, correctional setting and court system depending on the needs of the individual.
