Position Overview               

Are you looking for an exciting opportunity to be a part of dynamic team providing individualized services in the community? Great Rivers Behavioral Health Agency (BHA) is excited to announce the Cowlitz County School Based Mental Health Therapist position where you will have the ability to use your therapeutic skills to provide evidenced based therapy and the oversight of clinical services to children, youth, and their families with complex behavioral health needs in a school-based setting. Emphasis is on providing a whole-person approach to care, addressing physical and behavioral health needs in one system, through an integrated network of providers, who offer better coordinated care for patients and a more seamless access to the services they need. This position will work with the school facility, CIHS, community behavioral health agencies, hospitals, other specialty care providers, law enforcement, and other cross-system partners in Cowlitz County to ensure individuals with behavioral health needs are engaged in services.

Summary of Essential Duties and Responsibilities:

  • Identifies potential health problems through assessment of participants’ physical and mental status, daily living skills, coping patterns, strengths and supports.
  • Deliver medically necessary care coordination services to CIHS clients.
  • Interoperability of evidence based treatment plans and documentation for data collection and outcomes.
  • Will provide the data and medical history that will accelerate successful referral to psychiatric services.
  • Ensure annual age appropriate behavioral health screenings using evidence based tools.
  • Successfully transfer patient to long term counselor if and when appropriate.
  • Referral support and bi-directional tracking with communications to care team as needed for follow-up.
  • Clinical paperwork including documentation of all contacts, problems, interventions, determination of diagnostic eligibility, and dispositions of the case, including all referrals and termination summaries.
  • Submits referrals to community resources as needed using the school facility referral procedures.
  • Communicates and assists in the development of linkages between primary care providers, crisis services, community behavioral health agencies, social service agencies, law enforcement, family and/or other natural supports.
  • Identify methods of improving quality by reviewing clinical records of clients that have been seen by clinicians in the department and measuring data against standards identified by the agency.
  • Consultation with relevant community and state agencies, the coordination of multi-agency services, and participation in multi-agency support team on behalf of participants. Provides education and training to other individuals and/or agencies as necessary.
  • Provide ongoing measurement of individual in service outcomes and evaluation of intervention effectiveness. Address barriers and potential health problems that impede the development of skills necessary for independent functioning in the community.
  • Work with CIHS Program Director in establishing protocols and procedures to ensure standards are being met, and to help assist with implementing clinical protocols.
  • Provide case consultation with community services, mental health therapists, and family, as appropriate, and with consumer's written consent.
  • Constructively identify program needs and work with Director to enhance and develop programs to assure service coverage and continuity of care.
  • Be available to provide services in the community, as appropriate. Be continuously available during periods of coverage, including insuring that his/her location is known at all times and also how he/she can be contacted.
  • Participates in, and leads as necessary, quality improvement and assurance process for cases.
  • Provide consultation, education and training for CIHS staff and other agencies or individuals as deemed appropriate by the CIHS Director.
  • Perform other duties and projects as assigned by the supervisor.

Education Requirements: Graduate Degree

Salary Range: $45,000 to $65,000 annually


Job Requirements

Minimum Qualifications:

  • Master’s Degree in counseling, social services or closely related field.
  • Counselor registration in Washington State.
  • Two (2) years of experience providing direct treatment to the mentally ill and ability to meet the qualifications of a MHP per Division of Behavioral Health and Recovery (DBHR) regulations.
  • Proficient in computer use and applications commonly used or applied in this field/trade/position for the development of reports, documents, presentations and access/utilize databases for care management and Microsoft Office software.
  • Possess and maintain a valid driver’s license. Pass and maintain a clear background record as required for healthcare organization under state and federal contracts.

Preferred Qualifications:

  • Professional licensure as a LMHC/A, LMFT/A, or LICSW/A.
  • Child Mental Health Specialist certificate.
  • Experience working in a collaborative team while working with children and families in a school setting.
