Job Description

This position will deliver evidenced-based outpatient behavioral health and substance abuse therapy to individual, family, and groups to treat a broad array of behavioral health disorders with an emphasis on effective, outcome-oriented treatment, including substance abuse counseling and treatment.

 

Typical Work

- Identifies potential health problems through assessment of participants’ physical and mental status, daily living skills, coping patterns, strengths and supports.

-Consultation with relevant community agencies, the coordination of multi-agency services, and participation in multi-agency support team on behalf of consumers. Provides education and training to other individuals and/or agencies as necessary.

- Develops with the individual in service an assessment and comprehensive treatment plan that identifies the unique needs of each individual and may provide individual, couples, family, and group therapy. Participates in crisis intervention services with participants.

- Development and implementation of behavior plans for individuals in services with behavior problems due to personality disorders or cognitive deficits with clients and caregivers.

- Provide individual mental health, relapse prevention, harm reduction and substance abuse counseling based on motivational interviewing and cognitive behavioral responses to mental illness and substance abuse.

- Evaluate, on a continuing basis, services and expected outcomes. Be an advocate and liaison for individual in services in securing emergency detox and rehabilitation care.

- Document participant progress to maintain a permanent record of activity according to the agency's established methods and procedures.

- Assess and identify the treatment services individual in service individual in services based on severity of substance use diagnosis, stage of change and immediate needs, and recommend treatment objectives for the individual in service individual in service’s service plan.

- Assures that participants understand their rights to confidentiality and the limits of confidentiality, to include participant’s rights as recipients of mental health services and substance use treatment services.

- Provide individual supportive therapy, i.e. problem solving, role-playing, modeling and support, social skill development, and assertiveness training to increase client social and interpersonal activities in community settings.

- Provide ongoing measurement of individual in service outcomes and evaluation of intervention effectiveness. Address barriers and potential health problems that impede the development of skills necessary for independent functioning in the community.

- Completes and ensures the maintenance of all required clinical and administrative documentation.

- Represents the agency in coordinating service plans with other community providers.

- Participates in, and leads as necessary, quality improvement and assurance process for cases.

- Participates in a unified team and provides team coverage.

- Perform other duties and projects as assigned by the supervisor.

Education Requirements: Associates Degree

Salary Range: $40,000-$60,000 annually

 

Job Requirements

Minimum Qualifications (CDP):

  • Associate Degree in a clinical, human services, or related field.
  • Must possess a Washington State certification as a Chemical Dependency Professional (CDP).
  • At least two (2) years of clinical experience such as direct care, care coordination or case management in either public or private healthcare operations serving adults and/or youths.
  • Proficient in computer use and applications commonly used or applied in this field/trade/position such as Microsoft Office software and capacity to learn use of care management software.
  • Possess and maintain a valid driver’s license. Pass and maintain a clear background record as required by contractual requirements for healthcare organization under state and federal contracts.
